Appointments

To schedule an appointment, please call our office at (631) 265-0062, Monday-Friday between 9 a.m. and 5 p.m. On the day of your appointment, please arrive 15 minutes early to complete necessary new patient information forms. (To save time, please bring a list of your current medications and of your past medical history, so we can acurately document our records.)

Remember to bring a valid insurance card and photo ID. Also, please bring any referrals your insurance requires. If you need to cancel your appointment for any reason, please do so at least 24 hours in advance.

Insurance & Billing

For your convenience, Dr. Alpern accepts Medicare as well as most private insurances. Check the list below for your plan. Insurance plans are subject to change at any time, so please confirm your insurance when you make your first appointment.
